Franklin Community Health Network
Located in the western mountains of Maine, Franklin Community Health Network is an integrated network of rural healthcare providers. FCHN was created in 1991 by Franklin Memorial Hospital and joined MaineHealth, the state’s largest health care system, in 2013. The network now includes five affiliates:
- Franklin Memorial Hospital (FMH), a progressive, not-for-profit community hospital
- Healthy Community Coalition, with a coordinated public health approach of education, health promotion, and outreach to improve the well-being of the residents of greater Franklin County
- Franklin Health, a multi-specialty group practice sponsored by FMH and the largest provider of health care services in the region
- Evergreen Behavioral Services, providing 24/7 emergency mental health response and community outreach services in greater Franklin County
- NorthStar, the regional ambulance service of greater Franklin County with state-of-the-art equipment and modern ambulances
HealthReach Community Health Centers
HRCHC is a system of 11 medical practices serving the rural communities of central and western Maine by providing primary health care and outreach services to persons of all ages. Four of these eleven medical practices are located in the greater Franklin County region.
